Enrollment Checklist

Enrollment in the HPIN requires contractors to submit a signed 2026 HPIN Participation Agreement, Accountability Policy and HPIN logo use License Agreement along with the following documents:

Form W-9
Certificate of Commercial General Liability Insurance (COI)
EPA Section 608 Certification (Type II, Type III or Universal)
Active professional license held by employee or subcontractor (Master Electrician required; Master Plumber required for hydronic equipment)
Training certificate from a heat pump installation and design class provided by a manufacturer
